The Problem: For patients with osteomyelitis, septic arthritis, or pyomyositis, increased time from ED presentation to performing advanced imaging (MRI) can delay diagnostic and therapeutic orthopedic procedures (i.e., bone biopsy, joint aspiration, etc.).
Our analysis pointed to several key focus areas from our fishbone:
Project Results:
We were able to achieve our SMART of decreasing the time from ED to MRI.
This projects also had institutional impacts that include the following:
- Less wait time and total time in the ED
- Shorter MRI scan time
- General anesthesia / sedation not required, which means less personnel required.
Reflection:
There were several things that our team did well that lead to our success. The primary one being that we did a great job narrowing down the scope of our problem. Initially we tried to take on multiple issues but homed in on our scope to really be able to target one core issues.
In reflecting on this project, there were a few things we wish we had known. The team was surprised by the ripple effects that caused multiple teams involved with each patient that we didn’t anticipate. We also didn’t expect that it would be so difficult to make small changes! Nor did we expect adding a new order in EPIC to be as challenging as it was.
