The Problem: Receiving clearance forms and instructions less than 1 week in advance of surgery can result in time intensive and herculean staff efforts to avoid surgery cancellation or issues. This has implications for patient safety and cancer outcomes as well as resource utilization. We chose this project to focus on because of the direct impact on our workflow and was able to be intervened upon in a meaningful way.
Our analysis pointed to several key focus areas from our fishbone:

We were able to improve the percentage of clearance requests where this problem occurs.
This projects also had institutional impacts that include the following:
- Saved staff time
- Saved dollars of lost revenue
Reflection:
One of the most critical things to our success was that our communication amongst one another went well, and we had a good handle on the problem at hand. Initially, we were perhaps too focused on one aspect of the process, but in going through the class exercises, we realized there were multiple opportunities for improvement that could result in a better overall outcome. We wish we had a better sense of the time commitment for the course so we might have adjusted schedules differently. We also think it could be beneficial to have a “pre-course” consultation on the feasibility of the project and potential alternatives. The most surprising aspect was the seamlessness of the implementation and how quickly we’re seeing results. The course also went by so fast!
